A recent investigation by the Defense Nuclear Facilities Safety Board into worker safety and culture at the construction of the vitrification plant has stated that "Hanford's safety culture is so poor that it has 'a substantial probability of jeopardizing' construction and safe operation of the one-of-a-kind plant." The investigation was prompted by allegations that managers of the project refuse to take concerns raised by employees and others into account.
